To Change Your Mailing Address
Using your PERA PIN, you may log on to the secured Account Access section
and change your address online by clicking on Update Contact
Information. Please note: If a PERA benefit check is mailed
to your address, you must submit your address change in writing
and include your signature. If you prefer, you can also send your change of
address information by mail, fax, or you may call the Customer
Service Center.
To Change Your Beneficiaries,
Employment Information, or Name
Complete a Member
Information Form and return it by mail or
fax
to PERA.
If you are a participant in the PERA DC Plan
and would like to update your address, you should complete the
PERA Defined Contribution Plan
Change Form. A beneficiary change form is available on
the
DC Plan Web site.
To Claim Your PERA Account
If you received a PERA Account Warning Notice, you may claim
your account. (A refund packet may also be requested from this
link.)
