Active Member Plans
Active members are eligible to enroll in
PERACare if their employer has joined the program and they meet
the employer's eligibility requirements.
The Health Plan Descriptions (HPDs)
listed below have detailed plan information for the health care, dental, and
vision plans offered in PERACare. All employer plans are on
either a January or July renewal cycle. In order to select the
correct HPD for your employer's plan, you need to know your
employer's effective date or last renewal date.
PERACare HPDs for Active
Members Effective January 1, 2010, and July 1, 2010, effective
date or renewal date.
PERACare HPDs for Active
Members Effective January 1, 2009, and July 1, 2009, effective
date or renewal date.
